A single marketplace form can create several separate contact relationships. Treat the cleanup like a four-column project: organization, route, sent date, result.

This is general organizational help, not legal advice. Do not include medical records or identity documents in an initial request.

1. Inventory every contact

Review the seven days after you submitted the form. Search your call log, voicemails, texts, and inbox for senior-living communities, home-care agencies, and referral advisors. Record only identifiable organizations; do not guess who owns an unknown number.

2. Find an official route

Prefer, in order: a privacy-policy contact, a preference center, the organization’s customer-service email, or a direct reply to its message. Avoid sending personal information to an address you cannot verify on the organization’s own site.

3. Send one bounded request per organization

State that you revoke prior consent to marketing calls, texts, and emails. Ask for applicable deletion, confirmation, and any necessary identity-verification step. Keep the tone factual. A clear request is easier to route than a long complaint.

4. Track replies without over-sharing

  1. organization and verified destination;
  2. date sent;
  3. status: sent, verification requested, confirmed, declined, or follow-up due; and
  4. the next action and date.

5. Follow up once

If there is no response, reply in the same thread with the original sent date and ask for status. Privacy response deadlines and rights vary by state and request type, so use the organization’s policy or an official regulator source rather than a generic deadline.
